A clinical trial management system exists to be the operational system of record for a study: sites, monitoring visits, enrollment, payments, documents, and a trail that shows who did what and when. Those obligations do not come from a preference. They come from the fact that somebody outside your company may audit the study, and the record has to hold up.
Why buying is not a close call
Three reasons, and they are all boring, which is the point. First, validation. A system you built yourself has to be qualified for its intended use, and the effort to do that properly is larger than the effort to build the thing. Second, the audit trail is a feature with legal weight, not a log file. Third, everyone you work with, monitors, vendors, auditors, already knows what a trial system looks like, and a bespoke one costs you time in every single interaction.
People searching for free clinical trial management software are usually testing whether they can avoid this. Sometimes at a single-site investigator-initiated study you can. At a multi-site sponsor study you cannot, and the money saved on the license is spent twice over on the qualification work.
What building actually looks like
An internal build here does not fail at the tracker. It fails at everything around the tracker: access control that stands up, change history nobody can quietly edit, a documented specification, testing evidence, and someone who owns all of that in three years. That material lives in mail, and it stays in mail. If the record has to survive an inspection, buy the system. If several people have to work in the same record concurrently, buy the system.
